Case management services are provided by Care Navigators and other professionals who work with patients to identify barriers that may prevent them from reaching their healthcare goals. They work closely with our medical, behavioral health and dental providers while providing support and guidance to our patients. Care Navigators assist patients and families to navigate the often-confusing world of health care and numerous community resources to ensure their needs are met. This goes beyond physical and mental services to include assistance in obtaining social services provided by other community organizations such as food, housing, clothing, transportation and more. We participate in numerous outreach events that we conduct individually or in collaboration with community agencies such as immunization clinics, behavioral health symposiums, assistance with health insurance enrollment and much more.
