Insurance Opportunities
The Montana Legislature approved the Montana HELP Plan in 2016, offering affordable health insurance through expansion of Medicaid. The HELP Plan covers doctor visits, prescription drugs, emergency services, dental and vision services, chronic disease management and more. Greater Valley Health Center accepts Montana HELP Plan coverage and you may also be able to use our income-based sliding fee scale to cover remaining medical costs. The HELP Plan is open to Montana residents, ages 19 through 64, who do not qualify for Medicare coverage and who meet income guidelines. Our Care Coordinators can help you sign up for the HELP Plan or other insurance, or you can apply online at HealthCare.gov.

We accept all patients regardless of ability to pay. We have payment plan options, lower cost services, and a sliding fee discount for those who may qualify. Please fill out our sliding fee discount application and bring it with you to your next appointment.
Applying for Sliding Fees
To apply for our sliding fee, fill out the Sliding Fee Discount Program Enrollment form containing proof of your household income and family size. Examples of acceptable documentation of income include:
• One month of pay stubs for all employed adults
• Unemployment check stubs
• Social Security statement
• Most recent year’s federal income tax return
Reporting Changes
Applications for the sliding fee discounts are updated each year. Changes in income, or the size of your household, may affect your sliding fee discount and need to be reported as they occur.
